I understand that as a participant in the HistioUKConnect Go Champions programmes hosted by or on Histio UK various multimedia platforms and on-line communities hosted by Histiocytosis UK, I may:

• Submit my own video, audio for usage.
• I agree to allow Histiocytosis UK to keep, as their property, the products of such videotaping, audiotaping, interviewing, and/or photographing.
• I agree that such material, along with my name, may be used and posted on all brands pertaining to the Charity known as Histio UK, HistioUKConnect, Lets Talk and Histio Champions this includes website, social media, and other marketing or awareness materials for promoting the organisation and in any publication and/or publicity generated by the Charity Histio UK now and at anytime in the future. No compensation will be paid for such use.
• I hereby agree to release, defend, and hold harmless the Charity Histiocytosis UK and its agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper or via electronic media, from and against any claims, damages, or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion, or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction, or production of the finished product, its publication or distribution.
• I have read this release before completing the box which indicates and or substitutes for my signature, and I fully understand the contents, meaning and impact of this release.
